ATTENDEE WAIVER AND RELEASE OF LIABILITY

1. Assumption of Risk: I understand that my participation in all Altavida Club events involve inherent risks, including but not limited to:

• Use of the swimming pool, dance floor, and gym equipment

• Participation in wellness, fitness, and high-intensity classes (e.g., yoga, pilates, HIIT, running, etc.)

• Exposure to loud music and large crowds

• Consumption of food and beverages, including alcohol

• Physical activity in and around water, wet floors, uneven surfaces, and environmental/weather conditions.

  I voluntarily assume full responsibility for any injuries (including death), illnesses, losses, or damages sustained as a result of participating in this event, whether caused by the negligence of the hosts, venue, vendors, sponsors, or others.

  2. Release of Liability: I hereby release and discharge Glee Concept LLC, Ebanoda, Society Living, and all officers, agents, employees, contractors, volunteers, vendors, and sponsors from any and all claims, demands, or causes of action arising out of or connected with my participation in the event.

  3. No Liability for Third-Party Vendors: I understand that various vendors and sponsors will be present and offering goods and services. I acknowledge that these third parties are not employees or agents of Glee Concept LLC or Ebanoda, and I release the hosts from any liability arising out of vendor-provided products, services, or experiences.

  4. Health and Safety Acknowledgment: I certify that I am in good health and able to safely participate in the event. I acknowledge the risk of exposure to illnesses including COVID-19, influenza, and other communicable diseases at a public gathering. I agree not to attend if I am ill or symptomatic and understand that no health screening will be provided. I am aware that dehydration, overexertion, heat exhaustion, slips, falls, or sun exposure may occur and I take full responsibility for my wellbeing and hydration.

  5. Minors: If I am accompanied by a minor, I certify that I am their legal guardian and assume full responsibility for their behavior, safety, and compliance. I understand that wristbands or other means may be used to identify attendees under the age of 21.

  6. Alcohol Use Disclaimer: I certify that I am 21 years or older if I consume or accept alcoholic beverages. If I am between the ages of 18–20, I acknowledge I am not permitted to consume alcohol. If I violate this, I may be removed from the event without refund, and the incident may be reported to the authorities.

  7. Cancellation & Weather: I acknowledge that the event is subject to cancellation, delay, or rescheduling due to weather, public safety, or unforeseen circumstances. I understand no refunds will be issued in such events.

  8. Image & Likeness Release: I grant Glee Concept LLC, Ebanoda, vendors and third party companies at the event the irrevocable right to use any photographs, video, or likenesses of me taken during the event for marketing or promotional purposes without compensation.

  9. Legal Acknowledgments: I agree this waiver will be interpreted under Florida law. Any disputes shall be resolved in Orange County, FL. I agree not to initiate or join any class action lawsuit against the event organizers. I waive the right to a jury trial.

  10. Emergency Contact and Medical Release: In the event of an emergency, I authorize event personnel to contact the individual listed below and release medical information as necessary for emergency response
